Ride of Steel
In July 2011, U.S. war veteran James Hackemer, who had lost both his legs in a bombing during the Iraq War, fell to his death from the Ride of Steel at Darien Lake, a Superman-themed amusement park in New York. The ride initially closed down, but it was reopened after his death was determined to be a result of operator error; Hackemer should not have been allowed to enter the rollercoaster in the first place due to his physical condition.
