In June of 1986, tragedy struck at Galaxyland, an indoor amusement park in Alberta, Canada. The Mindbender ride was missing several bolts, which lead to a car flying off the tracks into a concrete pillar. Three people were killed, one sustained serious injuries, and nineteen others were minorly injured during the accident.
